Patient's Query

Hello doctor,

I am a 43-year-old male experiencing spots and discomfort on the head of my penis. This issue has been recurring over some time. Around two months ago, I suspected a fungal infection. Presently, certain spots have become more noticeable, particularly when I apply pressure to the head, and they appear as dark specks under the skin. I am currently using Butenafine hydrochloride and Hydrocortisone 1 % cream to manage the situation. Kindly help.

Hello,

Welcome to icliniq.com.

I read your query and understood your concern. Based on the information you provided and review of the images provided (attachment removed to protect the patient's identity), there seems to be a possibility of skin irritation stemming from repeated frictions, such as masturbation, or a reaction to substances used. It could also be linked to a vitamin deficiency or a skin condition like psoriasis. Fungal infection is an uncommon occurrence in your case as typically, it does not manifest as a discrete spot. Instead, it often appears as whitish material and tends to affect individuals with compromised immune systems, such as diabetics or those with HIV (human immunodeficiency virus) infections. Given the absence of itchiness or pain, there is likely no cause for significant concern. I suggest you refrain from using Cortisone cream, as it may exacerbate the situation. Instead, I suggest you prioritize maintaining good hygiene and ensuring the area remains dry and clean. I also suggest you use a non-chemical lubricant, such as olive oil, if engaging in masturbation. If you have recently changed your soap or shower gel, it could be a potential contributing factor. For the potential vitamin deficiency, I suggest you take vitamin E and vitamin A supplements. However, if you have a confirmed diagnosis of psoriasis, I suggest you use Methotrexate cream, although this is typically suggested only for individuals with a known history of psoriasis. I hope this has helped you. Kindly follow up if you have more doubts.

Thank you.

Patient's Query

Thank you doctor for the reply,

It does cause discomfort. The spots were concerning me. There appear to be dots on the head, especially on one side. Should I discontinue the antifungal and Hydrocortisone creams? Could they potentially be exacerbating issues like burning? Additionally, can the irritation result in an increased urge to urinate due to the irritation on the glans? While urination itself is not painful and there is no discharge, I do experience occasional soreness in my urethra and a burning sensation at times when not urinating. Kindly help.

Hello,

Welcome back to icliniq.com.

I read your query and understood your concern. I suggest you discontinue the antifungal and Hydrocortisone treatment. It appears to be a case of local irritation. I suggest you change your soap or shower gel instead of applying any substances. This change will help the situation resolve on its own without requiring further treatment. I also suggest you take a multivitamin that contains primarily vitamin A and Vitamin E for a duration of three months. Your issue will resolve completely and there is no need to worry. I hope this has helped you. Kindly follow up if you have more doubts.

Thank you.
