Hello doctor,

I have been suffering for the last three days with black color stool and weakness for one month. I had a CBC and stool examination and took some medicine from a local doctor. Please give your suggestions.

I am reviewing your case history and attached reports and will get back to you in a while.

I have reviewed (attachment removed to protect patient's identity) your case history in detail, and I have seen the attached report or blood profile, stool examination, and prescription. So, black stools are never normal, Some common reasons are bleeding from the stomach or small intestine, taking oral iron supplements, and consuming too much green leafy vegetables.

In your case, I have seen your blood profile; your hemoglobin is 15 g/dl, so less likely bleeding; otherwise, blood levels would have gone down. Check your medicine to see whether it contains oral iron or not.

Your stool examination is suggestive of brown stool (not black), but blood is positive, and I mentioned three reasons above - the first two can be the reason for occult blood in a stool sample. Pus cells are suggestive of stool infection, and you should be using medicine if you have loose stools at the moment. Tablet Rifaximin (antibiotic) 550 mg after breakfast and dinner for seven days will help you.

If you are already taking medication, wait for their response. Do not add further medicine. If you have persistent symptoms even after completion of your current medications, then use tablet Rifaximin as suggested.
