Hello doctor,

My doctor said my sac was measuring about 3 inches, but my report does not match what she said. I want a second opinion on the diagnosis based on the ultrasound report of a gestational sac measuring approximately 0.3 inches. She made a diagnosis of a blighted ovum.

Based on the ultrasound report you have provided, here is a second opinion. What your report shows is gestational sac size is 0.3 inches, and the yolk sac is present.

A blighted ovum (anembryonic pregnancy) is usually diagnosed only when there is no embryo visible in a gestational sac of at least 0.98 inches, or when a repeat scan in seven to fourteen days still shows no embryo. Your sac is 3 inches, which is far below the 0.98-inch threshold. The presence of a yolk sac is a positive sign and suggests that an embryo may be developing, but simply is not visible yet.

Based on your reported sac size, it is too early to diagnose a blighted ovum. At this stage, the correct classification is early intrauterine pregnancy of uncertain viability.

I would suggest the next steps for you are:

  • Repeat ultrasound in seven to 10 days to look for a fetal pole (embryo) and cardiac activity

  • Serial beta-human chorionic gonadotropin tests may be advised by your doctor to ensure levels are rising appropriately

The diagnosis of a blighted ovum is premature based on your scan. There is still a real possibility of a normal early pregnancy. Immediate action, like dilation and curettage or medication, is not recommended until follow-up confirms non-viability.
