What can be done for severe brain fog with a feeling of derealization?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hi doctor,

I am suffering from severe brain fog to the point where I cannot leave the house or go to work. I also have a dull, numb feeling on the top of my head. It almost feels like not enough blood is going to my brain. I have a feeling of derealization constantly. I have suffered from a feeling of derealization since I was about 15 years old but only in the last two years has it become debilitating. I have tried adding Potassium, Magnesium, Sea salt, B vitamins to my diet but nothing has worked. I am now trying the gluten diet and I am on day one. When I was 18 I took Ecstasy tablets most weekends for one year. But I have not taken them since then. I am 32 years old, 182.98 lb in weight, 5 feet 10 inches in height.

Hi,

Welcome to icliniq.com. To help you better I need the following details: 1. Have you got your thyroid checked ever? 2. How is your sleep? 3. Do you dream a lot in the night? 4. Do you feel tired the whole day? 5. Any anxiety related issues?

Patient's Query

Hi doctor,

Thank you for the quick reply. 1. I am not sure if they checked the thyroid but on the forums, I have seen people recommend this. What type of thyroid check should I get done? Would I get it from a general practitioner (GP) or do I need to so a specialist? 2. I sleep very well. I sleep nine hours per night usually. 3. Average dreaming at night I think. 4. I do not feel sleepy tired but I feel lazy. I do go to the gym but it is a struggle for some times. 5. This brain fog issue has led to social anxiety for me. I cannot be outside for too long. If I meet someone that I don't really know and they want to chat I get severe social anxiety. I used to be so outgoing but this has led to me being socially anxious.

Hi,

Welcome back to icliniq.com. Actually, hypothyroidism is one of the common reasons for brain fog. So, you can request your general practitioner (GP) for the test. It is a common and routine test. Other reasons could be excessive snoring that causes disturbed sleep and makes you feel foggy and tires the whole day long. And the last reason that is also very common is anxiety or depression in itself. So, in my opinion, you should get your thyroid profile done and if that is normal, your doctor may put your drugs that are of selective serotonin reuptake inhibitors (SSRI) class like Escitalopram, which helps in these symptoms. I would also recommend you taking morning walk, exercise.
