Hello doctor,
I am a 48-year-old woman recently diagnosed with metastatic breast cancer, which has spread to my liver and lungs. My original diagnosis was stage III breast cancer five years ago, treated with a mastectomy, radiation, and chemotherapy.
My latest mammogram shows recurrence in the remaining breast tissue, and my liver enzymes are elevated (ALT 85 U/L, AST 72 U/L), reflecting liver involvement. My oncologist is worried about how rapidly the cancer is progressing, and we have started aggressive chemotherapy again.
My blood counts are low (hemoglobin 9.5 g/dL, platelets 120,000/µL), and I am concerned about the side effects, especially fatigue and infections. The prognosis is uncertain, but my doctor is focusing on managing symptoms and extending survival.

Fatigue is one of the most common side effects of both cancer and its treatments. It can be worsened by anemia (low hemoglobin), which you are experiencing. Your oncologist may consider erythropoietin-stimulating agents or even blood transfusions.
With low platelets and hemoglobin, your immune system is more vulnerable. Take precautions like frequent handwashing, avoiding large crowds or sick individuals, and staying up-to-date on any necessary vaccines.
Elevated liver enzymes indicate the involvement of your liver in the metastatic process, which could be contributing to your fatigue as well. Monitor your liver function closely.
Targeted therapies, hormone therapy (if your cancer is hormone receptor-positive), and even clinical trials may provide additional treatment options. Connecting with cancer support groups can help. Please discuss any changes in your condition with your oncologist.
