Hello doctor,

I have shallow and fast breathing, usually bubbling and wheezing at bedtime along with nasal congestion and post nasal drip. My mucus is low and clear with a little cough. I have these symptoms for the past three months. I used Mucinex nasal spray, salt water spray, and Benadryl. I am a smoker for 30 years. I did not get any health problems so far. I have no fever but a little pain in chest with heart palpitations.

Welcome to icliniq.com. I can understand your concern. By your history and description, the possibility of bronchitis is more likely. It is commonly seen with smokers. It is the inflammation of airways. This inflammation causes bronchospasm which in turn causes wheezing, breathlessness, mucus production, etc. So please let me know, how many cigarettes you smoke in a day? Have you ever undergone a chest x-ray and PFT (pulmonary function test)?

I smoke 10 cigarettes daily. I have not undergone any chest x-ray and pulmonary function test (PFT). Can you recommend any medication?

Welcome back to icliniq.com. In my opinion, you should definitely consult a pulmonologist and get done clinical examination of respiratory system and PFT (pulmonary function test). PFT will not only diagnose bronchitis but it will tell you about the severity of the disease and the treatment is based on severity only. You will need inhaled bronchodilators (Formoterol or Salmeterol) and inhaled corticosteroid (ICS) (Budesonide or Fluticasone). And you need to quit smoking as early as possible.

Duolin Inhaler (Levosalbutamol plus Ipratropium bromide) 1-1-1-1 (one puff four times a day). 3. Foracort 200 mcg inhaler (Formoterol and Budesonide) one puff twice daily. All these for 15 days. Have you tried inhalers before? Do you know how to take inhalers?
