Hello doctor,

I was diagnosed with bruxism some time back. The previous year was stressful for me; from then, the problems started with feeling raised temperature in one of my ears, for which I consulted my dentist, who prescribed a dental splint, but it did not help and worsened the condition. I also have balance issues, weakness in my neck, and itchy ears. Kindly help.

I read your query and understand your concern. The bite guard you are already using is the treatment for bruxism. Use the custom-made dental splint to prevent the attrition of the teeth further. If the teeth are severely attrited, I suggest getting a crown over them, which will help restore the occlusion. I would also suggest using some of the muscle relaxants. Kindly revert in case of more queries.

I have gone through the X-ray (attachment removed to protect the patient's identity). From the X-ray, there is a possibility of infection with respect to the lower right wisdom tooth and lower right second premolar tooth that has already undergone root canal treatment. However, you have a problem on your right side, which could be from the infected tooth, for which I would suggest

  • Extraction of the right lower wisdom tooth.
  • Re-treatment for the lower second premolar tooth.
  • Tablet Augmentin (Amoxicillin clavulanate potassium) 625 mg three times a day for one week.
  • Painkillers if needed.
