Hi doctor,
Basically, I used to masturbate with soap. But now, the skin has turned rough and soft, and there are tiny bumps on the tip of the penis. I am very concerned about this. Please help.

Since your photo is consistent with scabies, the tiny bumps on the tip of the penis are most likely part of the scabies involvement rather than permanent damage from masturbation.
Scabies commonly affects the genital area in young males and can cause small itchy papules or nodules on the penis and scrotum. Using soap as a lubricant can definitely irritate the skin and make it rough, dry, or sensitive, but it does not cause permanent structural damage.
The roughness and softness you are feeling are most likely due to irritation combined with active scabies and scratching. The most important step now is proper scabies treatment.
Apply Permethrin 5 % cream to the entire body from neck down (including genital area, between fingers, under nails, buttocks, and soles) overnight for eight to 12 hours, then wash off in the morning. Repeat the same application after seven days.
All close contacts should be treated at the same time, even if they do not have symptoms, and clothes, bedding, and towels used in the last three days should be washed in hot water and dried in sunlight.
Avoid soap or harsh products on the penis for now, use only plain water and a mild, fragrance-free cleanser if needed, and apply a gentle moisturizer to help repair the skin barrier, like Vaseline petroleum jelly.
o not worry about the scabies regimen; it will get better in seven to 10 days. Also, if your skin feels irritated or burning, you can use 1 % Hydrocortisone cream at night on the affected penile area for five to seven days.
