Hello,

Welcome to icliniq.com.

I have read your query and can understand your concern.

Having burning in the penis for so many months, especially with negative STIs (sexually transmitted infections) tests, can be very stressful. Since your laboratory results are negative and you are already on Doxycycline, it is less likely that an active infection is the main cause. Burning during urination or when sitting for long periods can sometimes happen due to chronic prostatitis or chronic pelvic pain syndrome (CP or CPPS), urethral irritation, or even bladder-related conditions like interstitial cystitis.

These are not always picked up on routine STI or urine tests. Other possibilities include mild nerve irritation, pelvic floor muscle tightness, or even non-infectious inflammation of the urinary tract. The fact that this has lasted for nine months suggests it is more of a chronic irritation rather than an acute infection. Treatment in such cases usually goes beyond just antibiotics.

Doctors often use a combination of Alpha-blockers (example - Tamsulosin) to relax the prostate or urethra if prostatitis is suspected. Anti-inflammatory medicines to ease irritation. Do pelvic floor physiotherapy or relaxation exercises, since muscle tension can worsen the burning. In case of lifestyle steps, stay well-hydrated, reduce caffeine, alcohol, or spicy foods, and avoid sitting for very long periods without breaks.

I suggest following up with a urologist, since a prostate exam, urine culture, and sometimes a prostate fluid test or ultrasound of the urinary tract can help pinpoint the cause. The good news is that with proper diagnosis and tailored treatment, many men do find relief.

I hope this answers your query.

Let me know if I need to assist you further.

Thank you.
