Hi doctor,

Four days ago, boiling water burnt my buttocks. I had pain only on the first day. I use sterile gauze, Silver sulfadiazine, and a bacterial antibiotic cream or spray.

Recently, it started hurting while changing the dressing. What is the level of burn? I have attached a photo for your reference. What is the best treatment? Please help.

I read your query and can understand your concern.

It is a first-degree burn, as you can see the eroded skin (attachment removed to protect patient identity). You will need a systemic antibiotic along with topical care and daily dressings. Please stop using Bacitracin, as it can cause skin sensitization in many people. Instead, continue applying Silver sulfadiazine cream three times a day.

Make sure you do daily wound cleaning and dressing with Betadine lotion. In addition, the following injections are advised:

  • Piperacillin or Tazobactam 4.5 g IV, twice daily for seven days.
  • Metronidazole IV, three times daily for seven days.
  • Pantoprazole IV, once daily for seven days.

It is very important to stay hydrated. Drink at least 1.32 gallons (5 liters) of water per day. If you are unable to maintain this, you may also require IV fluids.

Eat a high-protein diet to support healing, and add plenty of coconut water, as it is rich in essential minerals and helps with recovery.

Yes, you can use it. But make sure to apply Betadine lotion right before dressing. Be careful, as I am talking about Betadine lotion, not ointment.
