Patient's Query

Hello doctor,

I am 25 years old. I have a 32-day cycle and my last period date was during the second week of last month. I had my beta hCG checked and the blood value was 16.80 mIU/mL. But the urine card test was negative. What does it mean? I read something about a chemical pregnancy. Is it something similar to that? How can I confirm?

Hello,

Welcome to icliniq.com.

I read your query and can understand your concern.

The card beta hCG is positive only if the value is above 25 IU. Values less than that are not significant. If there was a pregnancy, the value would be a lot higher. A chemical pregnancy is when beta hCG values are high, and the card test is positive, but there is no sign of pregnancy on ultrasound. That is not the case in your scenario. A value of 16.8 UI does not signify a pregnancy. For confirmation, however, you can repeat the beta hCG test 48 hours after the first one. If the values are rising, then you would need to do further tests. If not, then your period should start in a few days.

Hope this helps.

Thanks and take care.

Patient's Query

Hello doctor,

Thank you for your reply.

In the laboratory report, it was mentioned that less than 5 uIU/ml is not pregnant, which is why I had a doubt.

Hello,

Welcome back to icliniq.com.

I read your query and can understand your concern.

Yes, I understand the reason for your doubt. It would be best if you repeat the test after 48 hours as what is more important is the change in the two values rather than an individual value.

Hope this helps.

Thanks and take care.

Patient's Query

Hello doctor,

I rechecked after 48 hours and the result came as 65.29 mIU/mL. Is it pregnancy? Is there any chance of ectopic pregnancy?

Thank you.

Hello,

Welcome back to icliniq.com.

The repeat value after 48 hours is more than double the initial value, which favors a pregnancy. But given the duration since your last menstrual period, the value is very low for that gestational age. It just means that there are 2 possibilities

1. Delayed implantation of intrauterine pregnancy.

2. Ectopic pregnancy.

You would need to get a transvaginal ultrasound done with frequent monitoring.

Thank you.

Patient's Query

Hello doctor,

What monitoring should I do? When should I repeat the next hCG so that the gestational sac is visible?

Thank you.

Hello,

Welcome back to icliniq.com.

You need to get a transvaginal scan (TVS) done now, and it is not harmful to the fetus. The findings of the scan will determine the type of monitoring needed. If there are signs of intrauterine pregnancy, then you would just need to repeat the TVS. If there are no signs of pregnancy, then you might need to repeat both beta hCG and TVS. You would also need to get physically examined by a doctor including an internal examination.

Thank you.

Patient's Query

Hello doctor,

I had done TVS. No gestation sac seen intruterine and extrauterine. Ovaries have PCOD and hCG is repeated again after 48 hours from 65 mIU/mL it becomes 235 mIU/mL. Is that okay? The doctor suggested a follow-up scan after 2 weeks only. Is that enough?

Thank you.

Hello,

The rise in hCG is more than double, so it is probably suggestive of a growing intrauterine pregnancy. Yes, the ultrasound should be repeated only after 2 to 3 weeks to see any growth as before that the changes would not be seen. For now, you can just take Folic acid supplements and repeat the scan after 2 to 3 weeks.

Thank you.

Patient's Query

Hello doctor,

As I am too much worried I rechecked hCG after 48 hours and came as 749 mIU/mL. Is it a growing intrauterine pregnancy?

Thank you.

Hello,

Welcome back to icliniq.com.

As it is on the rising trend and the titles are more than doubling it is a good sign but only an ultrasound showing fetal cardiac activity will be able to confirm a growing intrauterine pregnancy. It will be best for you to wait and try to relax as stress is not good for the growth of the fetus.

Thank you.
