Hi doctor,

I am a male of 65 years old and have a weight of 183.01 pounds. Before ten months back, I started to have a tiny spot inside the eyelid and I tried using antibiotic drops. It does not help me and the doctor gave me Fusidic acid drops. It does not help me and I consulted another doctor. He gave me Chloramphenicol and it made the condition worse. I saw another ophthalmologist and told it as cysts. He gave me Maxicol steroid drops with Oral Doxycycline for three months. I have used it for ten days now and still, I have not got any relief. I am using a hot water bag for massaging and I am using Blephasol wipes and carbomer at night. When I wake up, I have crystallized discharge and it causes sharp pain. I am taking Lansoprazole and Thyroxine currently.

I have uploaded a picture but it was difficult for me to get a good shot. I thought that these treatments are antibacterial and so it has not worked for me. Is it a bacterial or fungal infection?

These are concretions (attachment removed to protect patient identity). They have to be removed by a needle. Even if you leave them, they do not cause any harm and it is just a foreign body sensation. I suggest you do a lipid profile. This condition usually occurs due to increased cholesterol. It gets blocked in the ducts and form crystals. An ophthalmologist can remove it and if it disturbs you, you can remove it. Otherwise, you can use lubricant eye drops alone.

Can I stop taking Doxycycline? Is it not due to bacterial infection?

No, these are concretions and they are not bacterial. There are meibomian ducts on the eyelids and it gets blocked due to improper drainage. Then they become crystalline and form concretions. They settle on their own and you can use lubricant eye drops for the foreign body presenting with a pricking sensation. Doxycycline does not have a significant role to play here. So you can complete the course and stop taking it. They have to be removed if it is present for a long time.
