Hi doctor,

Today, I found a blood clot after passing stools. I had constipation a few days back, and blood was seen at that time too, but I never noticed a blood clot before. I was suffering from dengue, but now my platelets are alright. I am attaching my recent reports. I am very worried. What is happening to me?

A dengue NS1 antigen positive confirms dengue viral fever. Your platelet count was within the normal range, which prevents you from bleeding. The symptom that you are talking about, blood clots with stools, has two options. One might be because of straining because of constipation, which sometimes bleeds or clots. It is generally associated with fresh blood.

The second chance is a bleeding manifestation due to dengue as a complication. That cannot be ruled out even though the platelet count was normal. With dengue, you need to remember that other bleeding manifestations, like rashes, bleeding gums, or blood in urine, might be associated.

If that was purely due to constipation, you would not notice any of these manifestations other than the blood clots or a little amount of bleeding. Also, let me know whether you have had any history of hemorrhoids or fissures in the anus. Did you have any mass per rectum? If you find any other bleeding manifestation other than the blood clots, I advise you to repeat the platelet count.
