I have a smooth, shiny patch on the glans of my penis under the foreskin. I sometimes get sores that are itchy under my foreskin, but they usually heal.

Regarding your symptoms and picture sent (attachment removed to protect the patient’s identity), I believe you have developed contact or allergic dermatitis from the possible use of soap, cream, or liquid that may have irritated the upper portion of your glans penis.
Since the allergic skin reaction is there, this may be constantly itching you. There is a high index of suspicion that this may be caused by chemicals from soap following frequent masturbation, since at this stage of age, you have not had sex.
So if you indulge in masturbation, I will advise you to stop or minimize it and kindly use the following topical medicines to help out: Topical 0.2% Tacrolimus cream or Topical Oit 0.05% Clobetasol should be taken once daily or 0.2% Hydrocortisone cream bid for one month.
