Hello doctor,

Today, after intercourse, I felt continuous bleeding and pain in the lower right abdomen area. It has not happened ever before. What is the reason behind it? I had taken contraceptive pills five days ago. I was diagnosed with a cyst in my right ovary two months ago through ultrasound. Please help.

Regarding your symptoms of right lower quadrant pain and bleeding after sex following a five-day history of contraceptives, I propose you have developed a possible differential diagnosis. The possibility could be a ruptured ovarian cyst, tubo-ovarian abscess, and ectopic pregnancy. Do you have any fever, vaginal discharge, vomiting? Please do a pelvic scan and schedule another appointment. Meanwhile, take the following medication:

  1. Tablet Acetaminophen 500 mg thrice daily after food.

  2. Tablet Metronidazole 400 mg thrice daily after food.

  3. Capsule Piroxicam 40 mg once daily.

  4. Capsule Co-Amoxiclav (Amoxicillin and Clavulanic acid) 625 mg twice daily after food.

  5. Tablet Medroxyprogesterone 10 mg once daily.

  6. Tablet Norethisterone 5 mg taken after food.

  7. Tablet Ketoconazole 200 mg once daily after food.
