Can RA lead to sternum and rib pain that affects breathing?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I am 54 and have rheumatoid arthritis. Lately, chest pain has been really worrying me. Deep breaths hurt, and the emergency room ruled out heart-related issues. My rheumatologist says it could be costochondritis, but the pain is quite intense.

Steroids seem to help briefly, but the pain returns. Sleeping has become difficult, and my anxiety makes everything feel worse. I am scared that every flare-up could be something serious. Being unable to breathe comfortably is frightening.

  • Can RA lead to sternum and rib pain that affects breathing at 54?

  • Is this common in people with RA?

  • How can I manage this without making constant trips to the emergency room?

Kindly help.

Hello,

Welcome to icliniq.com.

I read your query and understood your concern.

Yes, rheumatoid arthritis (RA) can cause inflammation of the cartilage that connects the ribs to the breastbone, a condition known as costochondritis. This can make deep breathing, coughing, sneezing, or certain movements quite painful and uncomfortable.

I would suggest the following:

  • Use warmth, such as a heating pad or warm shower, to help relax the chest muscles and reduce discomfort.

  • Pace your daily activities and avoid movements that consistently trigger or worsen the pain.

  • Practice slow, gentle breathing exercises to prevent chest wall stiffness and help reduce anxiety related to the pain.

  • Sleep with extra pillows or in a position that places less pressure on the chest.

Since steroids provide only temporary relief and the pain keeps returning, it is important to discuss your RA treatment plan with your rheumatologist. Recurrent symptoms may indicate that the underlying inflammation is not being fully controlled and that adjustments to your treatment may be needed.

You should seek urgent medical attention if you develop:

  • New or worsening chest pain.

  • Shortness of breath at rest.

  • Fever.

  • Dizziness or fainting.

  • Chest pain that feels different from your usual RA flares.

I hope you are satisfied with my answer. For further queries, you can consult me at iCliniq.

Thank you.
