Hi doctor,

I have been suffering from severe sciatic pain for the last three months and was reported to the hospital three months ago. An MRI scan was carried out two months before. I was diagnosed with degenerative disc disease or prolapsed intervertebral disc with issues in L3-L4 and severe problems at L4-L5 with significant compression of the L5 nerve root.

It has been causing severe pain in my left leg throughout the day and causing complete numbness at times. Currently, I am taking Pregabalin, Methylcobalamin, Gabapentin, and ayurvedic treatment. Kindly help.

I suggest you walk on your toes and then on your heels. I would like to know whether you can do that comfortably? Any bladder or bowel changes you have noticed for the last three months? Kindly revert in case of further queries.

Thanks for the reply.

I cannot walk on heels, such as applying a lot of pressure, which becomes painful, and the pain radiates in the entire left leg. There are no changes in bladder or bowel movements. Kindly help.

I understand your concern.

As mentioned in your magnetic resonance imaging (attachment removed to protect the patient’s identity) report, you said that you could not walk on heels, which means compression is very severe. In such a case, I would suggest surgical management. I would also suggest a temporary option in which a nerve root block is given, and you can get temporary relief which no one knows for how many days.
