Hello doctor,

I have left side lower abdominal pain and gas problem. I need a radiologist to interpret my MRI scan results. The laboratory tests performed was blood test in which CA 125 was elevated to 59.

Your MRI (magnetic resonance imaging) pelvis reveals (attachment removed to protect patient identity) a well defined hypo-intense lesion in the right adnexa from which right ovary is seen separately, and is likely to be an endometrioma. It shows mild homogenous enhancement on postcontrast images. However, your CA 125 levels are slightly higher for endometriosis. The usual cut off value is 30, but in extensive endometriosis, the values may rise. It is not like a malignant lesion as no fat component, or large cystic area or invasion is seen. Also, CA 125 values go above 200 in malignant lesions.

If possible can you please upload other investigations that you have done. Any ultrasound of pelvis, any surgical history, AFP (alpha-fetoprotein) levels, if done. And also any past reports which can be relevant.

I have seen your ultrasound report (attachment removed to protect patient identity). The right ovarian lesion they have mentioned as dermoid cyst. However, it is a benign pathology but if the pain is bothering you and affecting your everyday routine, I would recommend a surgical resection. Pelvis ultrasound and MRI in conjugation are a better modality for this diagnosis.

The pain I have is on the left lower abdomen. It is not on the right side. Could this pain be related to the above including the gas I feel in my stomach? How do I treat these conditions? Are they best if left alone?

The pain that you have on the left side can be justified due to vascular etiology secondary to the mass effect of the right ovarian lesion. If you can get a pelvic doppler done, it can be diagnosed with that. However, if that is the cause, then again removal of the dermoid will relieve you from pain.

Same symptoms don't mean you have the same problem. Consult a doctor now!
