Can high TSH and a thick endometrium affect periods?

Hello doctor,

My period was delayed, and I experienced occasional blood stains in my urine. But now, I have finally gotten my period after approximately 20 days. Following the blood test and ultrasound, I understand that my B12 levels are low, TSH is high, and my endometrium is thickened to 0.67 inches.

Also, I had spotting, three weeks before my period, every second or third day. My period got a 20-day delay. I am experiencing hair fall, weakness, and a little bit of body ache for the last one and a half months.

While my family doctor advised waiting for one month and repeating the ultrasound after my next menstrual cycle, I am feeling quite stressed and anxious about the situation. I am concerned that there could be something serious, and I am wondering if it is safe to just wait or if further investigation or consultation is recommended.

Endometrial thickening (0.67 inches or 17 mm): For someone who is menstruating, the endometrial thickness can vary across the cycle, and 17 mm can be seen during the secretory phase (post-ovulation, before the period starts). However, when irregular bleeding or delayed periods are involved, especially spotting for weeks, it is important to rule out endometrial hyperplasia or other pathology.

Waiting for one cycle and repeating the ultrasound is standard practice, especially if the endometrial lining may shed during the next period. However, if your next period is again delayed or the spotting continues, or if the thickness persists, a referral to a gynecologist and possibly an endometrial biopsy may be advised.

2. High TSH (thyroid-stimulating hormone): High TSH often means hypothyroidism, which can cause irregular or delayed periods, spotting, hair loss, fatigue, weakness, and body aches. If not already done, T3 (triiodothyronine) and T4 (thyroxine) levels should also be tested to confirm thyroid function. If hypothyroidism is confirmed, starting thyroid hormone replacement (Levothyroxine) is likely to help regulate your cycle and reduce other symptoms.

3. Low vitamin B12 can cause fatigue, body aches, weakness, and sometimes neurological symptoms like tingling or memory issues. You may need B12 injections or high-dose oral supplements, depending on how low your levels are and whether you have absorption issues.

  1. Follow-up on thyroid tests (if only TSH was checked, get T3/T4 as well).

  2. Consider starting treatment for hypothyroidism and low B12 immediately under your doctor’s supervision. These two could explain many of your symptoms.

  3. Track your next cycle carefully. If your period becomes more regular and the spotting resolves, that is reassuring.

  4. If spotting continues or the endometrium remains thick, a gynecological referral is appropriate for further evaluation, possibly including a biopsy.

  5. If you are very anxious, consider requesting a repeat ultrasound sooner or seeking a second opinion from a gynecologist now, especially to ease your stress.
