Hello doctor,
A 1.69-inch dermoid cyst was found on my right ovary during an ultrasound. My left ovary is clear. My doctor advised me to try to conceive first, as I am currently trying to get pregnant. My CA-125 level is 77. Is it normal for CA-125 levels to rise with a dermoid cyst? Will I be able to conceive with this cyst? My doctor also prescribed folic acid for me.

As you mentioned, the ultrasound shows a dermoid cyst on your right ovary, measuring 1.69 inches. A dermoid cyst is a benign tumor with a very low chance of becoming malignant. The risk of malignancy in premenopausal women is approximately one in 1000, increasing slightly to three in 1000 by the age of 50.
Yes, CA-125 (cancer antigen 125) levels can be mildly elevated in the presence of a dermoid cyst. The normal range is typically under 35, and in your case, it is 77. This mild elevation is not usually a cause for concern, especially in the context of a known benign cyst.
Since the cyst is less than 1.9 inches and you are currently asymptomatic, no immediate intervention is necessary. A follow-up scan (preferably a transvaginal ultrasound) is recommended after four months to monitor for any changes in size or other characteristics.
You can still conceive with this cyst, especially since your left ovary is normal. Please continue the medications prescribed by your doctor. At this stage, the cyst only requires observation, unless it becomes symptomatic or shows significant growth.
