Hello Doctor,

My 35-year-old friend has type 1 diabetes and was recently diagnosed with hypothyroidism. Her doctor mentioned that managing blood sugar levels can become more challenging with both conditions. She is currently on levothyroxine 50 mcg daily for her hypothyroidism. Still, she is experiencing frequent blood sugar fluctuations, with her most recent HbA1c being 8.1 percent and fasting glucose levels ranging between 170 to 200 mg/dL. She follows a basal-bolus insulin regimen, including Lantus twelve units at bedtime and Novorapid with meals. She also takes Atorvastatin 10 mg daily for cholesterol, and her blood pressure is managed with lisinopril 5 mg. She is concerned about weight gain associated with hypothyroidism, which she feels might further impact her ability to control her blood sugar levels. She is wondering if her thyroid medication might be influencing her insulin needs and how to better balance both conditions. Could you kindly provide guidance on how to manage both Type 1 diabetes and hypothyroidism effectively, particularly insulin adjustments and potential weight management strategies?
