Hello doctor,

I am 13 weeks pregnant for the first time. I have a gas issue and loose motion with stomach pain hence my doctor gave me Metrogyl 200, Happi D, and Loperamide. Are they safe to use during pregnancy period? Kindly suggest.

Throughout your 40 weeks of pregnancy, the baby's body parts are growing. Especially in the first half of pregnancy body parts are forming. So any medication should be decided with a lot of precaution. Try to avoid any medication as far as possible. Most of them can traverse through the placenta and may be harmful to the baby. Diet supplements like iron, calcium, vitamins, and proteins are acceptable. Anything other than this can harm the baby. But in dire necessity, you have to choose among the safest. So coming to your prescription

1. Metrogyl (Metronidazole) 400 mg is an antibiotic of the adult dose. Not 200 mg. Antibiotics are advised in diarrhea or loose stools only if associated with fever. If it is just indigestion, antibiotics are of no use. So unnecessary in your case. Pregnancy is always associated with gastritis, which only needs reducing spicy, oily food. Having lots of curds, milk, juice so on.

2. Happi D is Pantoprazole with Domperidone. Pantoprazole is acceptable in pregnancy if gastritis is severe and taken only when needed. Not regularly. Domperidone is antiemetic, stops vomiting. But you never mentioned it. So again unnecessary. You can have Pantoprazole 40 mg alone before food when required.

3. Loperamide is antidiarrheal. It is a banned drug. It just stops the diarrhea by contracting the sphincter around the bowel exit. If you have indigestion due to some food, which your body is not accepting and trying to throw out, protecting your body, is not stopping that diarrhea wrong. So let your body throw it out, so this medicine is unnecessary and harmful to you and your baby. All you need to do is avoid heavy food, spicy, oily food. Reduce non-vegeterian food. Have homemade food only. To compensate for the water and salt lost through stools, have lots of water. Better if you can have lemon juice with salt and sugar, coconut water, or any juice with salt and sugar or ORS (oral rehydration solution). You will feel rejuvenated.
