Hello doctor,
I am 60 years old. I have pressure on the front teeth above the gum line for about 1/4 to 1/2 inch. The dentist thought it was due to replacing caps with new ones, affecting the bite and causing pain on the gum above the tooth.
My gums are tight, and I have visited an endodontist for a second opinion. X-rays again show infection. My teeth feel tight, and today, between the front teeth, a small whitehead appears and pops like a small pimple. It is not an abscess, no foul odor, etc. I am rinsing with saltwater and peroxide. It is annoying.

The cause can be a faulty prosthesis. You should undergo IOPAR (intraoral periapical radiography).
As per your detailed query, you have apical periodontitis. This is due to the high points on caps. When you close your teeth, your opposite side puts pressure on the teeth, which are now becoming annoying. Because the pressure is at the tip or apical portion of the root, you feel pain inside the gums, and there is now a draining sinus.
You need to take antibiotics and pain killer like NSAID (non-steroidal anti-inflammatory drugs) because they help in inflammation.
You need to take medication and removal of the prosthesis. Take tablet Augmentin (Amoxicillin and Clavulanic acid) 625 twice daily for five days, Dolomec SP (Aceclofenac, Serratiopeptidase, and Paracetamol) BD for five days. You should avoid hard foods.
My suggestion is to either have the cap removed or trimmed so that the high point, which is causing it, will go off.

Then you need to visit a periodontist because it is due to stress. When there is more pressure on your periodontium (tissue around the tooth root), there are chances of inflammation.
My opinion is to take the above medication for five days, at least Augmentin and get your crown readjusted as soon as possible. There is also no need to worry because it is not any dangerous situation. It will heal within days after taking medicines and getting your prosthesis adjusted.
