I see that you are on hypertensive medications. For how many years are you taking Propranolol?

This question is extremely relevant to my diagnosis, and most importantly, I want to know if you are getting ear pain only while chewing or eating or when you bite food. I will accordingly prescribe and suggest remedies for correcting joint-related ear pain due to the bridge pressure.

I have been taking Amlodipine 4 milligrams antihypertensive for blood pressure for only three months. And Propranolol for headaches for three years. I get ear pain only once or twice a week with or without chewing or eating food. I am getting ear fullness when bending over.

I can safely prescribe this tablet called Hifenac MR (Aceclofenac 100 milligrams, Chlorzoxazone 500 milligrams, and Paracetamol 325 milligrams). Once a day for up to three to five days whenever your joint or ear pain aggravates. It will help. As for your symptom of ear fullness when bending over, it is unlikely to be related to your zirconia bridge or bite. If you are bending and there is an ear issue, then it is a case to be investigated by an ear, nose, and throat (ENT) specialist in the clinic. I would recommend an in-clinic ENT check-up. Also, you can ask your local dentist to trim a bit of the bridge where you bite down. Trimming the bridge can allow joint comfort and reduce your ear fullness.

Yes, you can definitely trim the zirconia bridge. Though you may have a light risk of being slightly less strong than earlier (like when it was fixed), it is still more biocompatible with your teeth than a ceramic bridge. Definitely, light trimming can be done over zirconia, which can relieve a bit of joint and ear pain. Try asking your dentist to do that for you. But ideally, after this as well, ear fullness is an issue that needs to be addressed by the ENT specialist. The ear canals need checking.

I had a regular doctor check with light and that small type of camera. It is good to hear that zirconia is a good choice. I will give it a try and see if he can trim it. I have a bit of a problem with the lower left side tooth. The dentist has advised he wants to take the cap off and perform a root canal. My previous dentist said to leave it for a year and see how it does. What do you think? Not painful, but it does have a small cavity in there somewhere.

2. Recap the bottom left tooth first without a root canal. Is it okay?

I agree with this treatment. Cap without a root canal is ideal when you do not have any sensitivity or pain in that tooth. Do you have a sensitivity or not?

Around the bottom left tooth, where the cap is displaced, I can see that bone loss is there. Can you tell me when you last got a dental cleaning done? Did they do dental scaling for you?

Where do you see the bone loss occurring at the root end?

This cap was replaced one year ago. As of now no pain or sensitivity to hot or cold but a bit of galvanic shock or oral galvanism. Yes, they cleaned my teeth about three months ago.

The bone is receding in between the first and second molars. I feel you should go with the new cap, and if you are not having any issues like pain or sensitivity in the lower last left tooth, there is no need for a root canal as such. Just go for a new zirconia cap. Dental scaling you keep getting done every three to six months is good to prevent vertical bone loss.

Get the zirconia trimming done soon, and also, please go for an ENT check-up before you proceed dentally ahead. You will know the findings, if any.
