Patient's Query
Hi doctor,
I have been having this feeling of imbalance for six years during lockdown; sometimes my head is light, and sometimes tense. The problem is that the feeling of imbalance does not go away. The laboratory test and CT (computed tomography) scan are normal
Please help.
Thank you.
Hello,
Welcome to icliniq.com.
I am deeply concerned about your worries.
Based on your history of persistent imbalance for six years, a normal CT (computed tomography) scan, normal blood pressure, and largely normal laboratory tests, the most likely possibilities include the following:
Persistent postural-perceptual dizziness (PPPD), a chronic dizziness disorder characterized by a constant sensation of imbalance, lightheadedness, or unsteadiness despite normal imaging and routine tests. Symptoms often begin after a stressful event, illness, or prolonged anxiety.
Anxiety-related chronic dizziness or functional dizziness.
Vestibular disorders affecting the inner ear, such as a previous episode of vestibular neuritis that evolved into chronic imbalance.
Less likely, given the long duration and normal CT (computed tomography) scan, are serious neurological disorders.
Your cholesterol profile is generally acceptable, with a high HDL (“good” cholesterol) level. Blood pressure and pulse are normal.
I recommend evaluation by an ENT specialist with vestibular testing and a neurologist if it has not already been done. An MRI (magnetic resonance imaging) of the brain (if never performed) may be considered, as CT scans can miss some conditions affecting balance.
The most likely diagnosis is persistent postural-perceptual dizziness.
To better assess the cause. Check if you experience the following symptoms where you feel worse:
When standing.
Walking in supermarkets.
Looking at moving objects.
Scrolling on a phone.
Being in crowded places.
These are common triggers of PPPD.
I hope it helped with the query.
Please let me know if you have any more questions on your mind.
I would be happy to guide you.
Thank you.
