Q. MRI of IAC for asymmetrical hearing loss shows small vessel disease. Please explain.

Hello doctor,

I am a 56-year-old male. I recently had an MRI of IAC and brain because I have asymmetrical hearing loss and tinnitus in my left ear. No acoustic neuromas found. However, I was scared when I found out that MRI showed mild chronic small vessel disease. I also read online something about people having strokes with two years of hearing loss, such as mine. My ENT is treating me for the hearing issue and my primary said that small vessel disease is common and he has it himself. He just said to keep my blood pressure under control as well as cholesterol. Exercise daily, eat a healthy diet and lose some weight. I am not sure what else I should do to protect myself or should I see a neurologist? I am currently on Bystolic 10 mg and Atorvastatin 40 mg daily.

I understand your anxiety. Mild small vessel disease is common in people older than 50 years. I agree completely with your primary physician's advise. Additionally, I would like to add that stop smoking if you do.

As you said, the development of stroke after hearing loss this is not true, in fact there is no relationship with stroke and hearing loss. I do not think you need to see a neurologist. This is age-related change.

I appreciate your input and it put me more at ease. I am debating on whether I should take a low dose 81 mg Aspirin daily. I know it has advantages but risks as well. What would you recommend?

I do not recommend Aspirin for this reason. The above said measures of controlling blood pressure and sugar, keeping cholesterol in normal range, healthy lifestyle, etc., are sufficient. There is no added advantage with Aspirin.
