Hello doctor,

I have dry skin on my legs despite proper hydration and moisturizing. Not itchy, no redness. Just rough, dry skin. How to treat this?

I am really sorry for what you are going through, and I can completely understand your situation. The condition you describe, persistent dryness and rough skin on the legs without itching or redness, is usually due to skin barrier dryness, sometimes called xerosis, where the outer layer of the skin loses moisture and natural oils. This can happen even if you drink enough water, because hydration from inside does not always correct the dryness of the skin barrier.

You can improve this with a simple skin-care routine:

  • Use a mild moisturizing soap while bathing, as harsh soaps can strip natural oils and worsen dryness.

  • After bathing, gently pat the skin dry and apply petroleum Jelly over the legs twice daily, especially immediately after showering, to lock in moisture.

  • Moreover, use a lotion containing Lactic acid and urea once daily for about two weeks, as these ingredients help hydrate the skin and smooth rough texture. Options include lactic acid lotion or urea plus Lactic acid cream, which help gently exfoliate dead skin and improve moisture retention.

Also, try to avoid very hot showers, limit bathing time to about ten minutes, and wear soft cotton clothing to reduce friction on the skin. With regular moisturizing and keratolytic lotions, the roughness usually improves within a couple of weeks.
