Why does a toddler have smelly ear discharge?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

My baby, who is two years old, has had mucus coming out of his ear with a smell for the past six months. I have used antibiotics along with vitamin C and Ibuprofen. However, I noticed a light bloodstain today.

Please help.

Thank you.

Hello,

Welcome to icliniq.com.

I have gone through your query and understand your concern.

Any discharge from a child's ear, especially when accompanied by a foul smell for six months and now a trace of blood, is an urgent red flag that requires immediate medical evaluation.

At this age, such symptoms often indicate a chronic inflammatory process in the middle ear that has likely caused a perforation (hole) in the eardrum, creating a gateway for deeper infection. The fact that previous treatments with antibiotics and vitamins have not resolved the issue suggests that the infection is either highly persistent or resistant to those medications.

The primary risk is not just the discomfort but the potential for permanent hearing loss due to prolonged inflammation, which could significantly impact your child's speech and language development in the future.

Please do not attempt to clean inside the ear or use any ear drops until a specialist confirms the integrity of the eardrum, as many common medications can be toxic to the inner ear if they pass through a perforation.

I hope I have answered your question.

Let me know if I can assist you further.

Thank you.
