Why does eczema on the thumb and index finger keep returning?

Hello doctor,

I have been suffering from eczema or fungus on the right thumb and index finger. It looks like it is expanding, and it leaves apparently dead skin behind, which peels off. I have attached pictures for a better understanding. It itches mildly to moderately, and it hurts slightly when pressure is applied.

I thought it might be an allergic condition due to my hands touching the steering wheel 12 hours a day, but it is happening only in my right hand. It itches mildly to moderately, and it hurts slightly when pressure is applied. Occasional small eczema-type skin condition in the same present affected area, but they used to come and go. This time it looks like I have also got a fungal infection, or so besides the original condition, which would come and go, without any major skin damage. I use over-the-counter ointments like hydrocortisone, colloidal oatmeal, and antifungal cream.

I can understand your concern.

From your pictures (attachment removed to protect patient identity), it is more in favor of eczema, but it is also infected. Do you use gloves while holding the steering wheel? It is not a fungal infection clinically. You have to apply a combination ointment containing Betamethasone and Neomycin. The most important thing for eczema is to maintain moisture and hydration of the skin. For that, you have to use Vaseline (petroleum jelly) four to five times a day. Take a non-sedating antihistamine. Maintain the hygiene of the area. Wash with soap and water, and avoid using Dettol (Chloroxylenol) and other irritants. You will get better.
