Is it safe to consume a gluten-rich diet before endoscopy?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

My 5-year-old daughter will have an endoscopy to check for celiac disease next week. I give her everything in her diet; some days, she eats bread and pasta with gluten, but some days, she eats rice and non-gluten food. I learned today that she needs to eat gluten every 14 days before the endoscopy; I have not been deliberately giving her gluten every day.

She has been eating pizza bread with gluten on certain days and rice and potatoes on others. Now that there are just six days till the endoscopy, I will give her gluten actively every day. And, up until now, she has eaten gluten on some days but not others. Is this sufficient for an endoscopy? Will we still receive the accurate result?

Thank you.

Hello,

Welcome back to icliniq.com.

Yes, it is enough. Do not worry; you will get an accurate result. If you can give her a detailed history and earlier investigations, it will be helpful for me to advise you further.

I hope this helps.

Thank you, and take care.

Regards.

Patient's Query

Hello doctor,

Thank you for the reply.

So, why does the paper state you should eat gluten every 14 days before an endoscopy? I saw that she was not consuming gluten on the weekend. These three days are within the fourteen-day timeframe leading up to the endoscopy. Are you sure that the result is still accurate even if we did not give her gluten for three days?

Her past medical history:

She is five years old and has suffered from constipation and sometimes stomach aches for the past 2.5 years. Her weight gain takes place slowly, and she eats infrequently without an appetite. Many members of my husband's family, including my mother-in-law, suffer from celiac disease. We reviewed the blood test for celiac disease, but the results were negative.

We tested for calprotectin three times: once, it was a positive result (69), and twice it was less than 5. Because we were unable to solve the problem, the doctor wishes to perform an endoscopy to determine the presence of celiac disease.

We gave her a regular diet that includes gluten, and I have never tried it without it. However, after three days without giving her gluten, I am not sure whether I will be able to have the endoscopy next week. Even though she has not eaten gluten for three days, will the test results show that she has celiac disease?

Kindly help.

Hello,

Welcome back to icliniq.com.

Although there is never much room for error, there are some precautions that should be taken for optimal conditions. Three days off does not matter.

I hope this helps.

Thank you and take care.

Regards.
