Hello doctor,

I have had a deeply embedded pimple on the right side of my forehead for several years and was never able to remove it. All of a sudden, this week, it became inflamed and puffy. I used a needle on it, and a large amount of pus, blood, and waxy material came out. I thought it had been completely removed. Now my forehead is swollen, and my right eye is swollen shut. I do not understand what is happening or what to do about it.

Based on the description, it is most likely that there was a long-standing epidermoid cyst (sebaceous cyst) or a deeply embedded acne cyst on the forehead that has now become acutely infected. The release of pus, blood, and waxy material after puncturing the lesion strongly supports this diagnosis. Unfortunately, puncturing or squeezing such lesions at home often pushes bacteria deeper into the skin, causes rupture of the cyst wall beneath the surface, and allows the infection to spread into surrounding tissues instead of resolving.

The rapid swelling of the forehead, along with swelling of the right eye to the point of closure, is a warning sign. This raises concern for cellulitis, with possible preseptal or orbital involvement. The forehead, nose, and eye region form what is known as the danger triangle of the face, where infections can spread rapidly because of shared blood and lymphatic drainage. At this stage, the condition is no longer limited to the skin.

At present, the area should not be squeezed, punctured, or manipulated again, as this can worsen swelling and push the infection deeper. Heat should not be applied. Gentle cleansing with mild soap and water is sufficient. The presence of fever, increasing pain, spreading redness, changes in vision, pain during eye movement, or increasing tightness around the eye indicates an urgent situation.

Given the current symptoms, an immediate in-person medical evaluation is necessary, preferably the same day. Treatment may require oral antibiotics or intravenous (IV) antibiotics, and in some cases, proper surgical drainage under sterile conditions performed by a medical professional. Delaying treatment increases the risk of serious complications.

If possible, a clear photograph can help in assessing the extent of involvement while medical care is being arranged. Ideally, one photograph from the front and one from the side should be provided, showing the forehead swelling, eye involvement, and any redness spreading toward the nose or temple.
