How can I manage my erection issues during sex?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hi doctor,

I am getting proper morning erections, and during foreplay, my penis only becomes erect when I rub it. The erection immediately goes away when I stop rubbing. As a result, I cannot engage in real sex.

I also took Tadalafil 5 mg (on my doctor's suggestion) one hour before sex, but there was no improvement. Could you please help me? I have been taking Amlong 5 mg for six years.

Please guide.

Thank you.

Hi,

Welcome to icliniq.com.

I read your query and understand your concern.

According to your complaints, you may be experiencing erectile dysfunction (ED). Potential reasons for erectile dysfunction (ED):

  1. Stress: Pressures from work or family, as well as night shifts, can contribute. It is advisable to plan for regular exercise and yoga.

  2. Varicocele: An ultrasound of the scrotum can help diagnose it.

  3. Fear of failure: Experiencing ED even once may lead to anxiety about future occurrences.

  4. Low self-esteem: This may be linked to chronic masturbation.

  5. Sexual orientation: Lack of attraction to the same sex can be a factor.

  6. Substance use: Smoking and alcohol consumption can contribute to ED. Long-term illness: Conditions such as cardiac disease, thyroid disease, kidney disease, or any illness requiring long-term medication may play a role.

  7. Vitamin deficiency: Simple deficiencies can be addressed with supplements like tablet Zincovit (multivitamin) and Shelcal (Calcium and Vitamin D).

  8. Negative attitude towards your partner.

  9. Hormonal issues: Conditions such as thyroid problems, low testosterone, or increased prolactin levels may cause ED.

  10. Diabetes and hypertension.

It is advisable to check for:

  1. Doppler ultrasound of the penis.

  2. Ultrasound of the pelvis and scrotum.

  3. Blood pressure.

  4. Random blood sugar (RBS).

  5. Free T3 and T4 levels.

  6. Testosterone levels.

  7. Thyroid-stimulating hormone (TSH).

Please share more details regarding the above reasons so we can work on a diagnosis and treatment plan.

Lifestyle recommendations:

  1. Exercise: Aim for regular physical activity and a balanced diet.

  2. Basic exercises: Kegels, squats, yoga, running, or brisk walking for five minutes.

  3. Recommended foods: Include strawberries, apples, avocados, sweet potatoes, sesame seeds, almonds, garlic, and a small amount of chocolate in your diet.

For a temporary solution, you can use tablet Mildfil (Tadalafil) 5 mg twice a day. If you prefer to take it only before sexual activity, a dose of 20 mg should be taken approximately 30 minutes before intercourse. Please let me know if you need further assistance.

I hope this information is helpful.

Please let me know if you have any further questions, and I would be happy to assist you.

Thank you.

Patient's Query

Hello doctor,

My foreskin does not retract naturally during erection. Will it cause any issues during penetrative sex? Do I need any treatment for this?

Please help.

Thank you.

Hello,

Welcome back to icliniq.com.

I understand your concern.

I can understand that if your foreskin is not naturally going to the normal position, it means phimosis. If the phimosis happened recently, medication is available for the cure; if the phimosis is caused by birth, surgery (circumcision) is the only option.

I hope this helps.

Kindly follow up if you have more concerns.

Thank you.

Patient's Query

Hello doc,

My foreskin does not retract naturally to show to head of the penile glands during erection. Should Does foreskin not cover the head of the penile glands during penetrative sex?. Will it cause any issues during penetrative sex?

Please help.

Thank you.

Hello,

Welcome back to icliniq.com.

During sex foreskin will retract fully without pain. It is difficult to have sex with a closed foreskin. When you go into the vagina automatically the foreskin will automatically go back, your glans penis will be exposed, and you will feel the excess sensation. If the foreskin is closed during sex, painful, less enjoyment during sex.

I hope this helps.

Kindly follow up if you have more concerns.

Thank you.
