Why is there facial swelling and red eyes even after Benadryl?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

Recently, I have experienced swelling on the left side of my face, extending from my eye to my chin, including noticeable puffiness and redness under the eye. To address this, I took Benadryl at last night and the next day morning; however, I realized it expired in three months ago. Despite this, the swelling remains unchanged this morning.

Please help.

Thank you for your assistance.

Hi,

Welcome to icliniq.com.

I read your query and can understand your concern.

Thank you for providing detailed information about your symptoms. Based on your description, persistent facial swelling, redness, warmth, and lack of improvement despite antihistamine use, it appears this may not be a simple allergic reaction. Several factors raise concern:

  1. Duration: The swelling has persisted for over 48 hours.

  2. Unilateral presentation: The swelling is confined to one side of the face.

  3. Signs of inflammation: There is warmth and redness, indicating possible infection.

  4. Systemic symptoms: You have experienced body aches and a feverish feeling, suggesting a systemic response.

These symptoms are concerning for a localized infection, such as cellulitis or a sinus-related abscess (swelling with pus in the sinus region), particularly if the area under the eye and cheek is involved.

Recommended actions:

  1. Seek urgent medical attention: It is crucial to consult a healthcare provider today. Facial cellulitis can progress rapidly and, if untreated, may affect the eye or even the brain in rare cases.

  2. Medical evaluation: A healthcare provider will likely:

    1. Conduct a physical examination.

    2. Possibly order blood tests or imaging studies such as a CT (Computed Tomography) scan) if an abscess or sinus involvement is suspected.

    3. Prescribe antibiotics, which may be oral or intravenous, depending on the severity.

Interim measures:

  1. Avoid manipulating the area: Do not press or massage the swollen area.

  2. Continue antihistamines: You may take a non-sedating antihistamine (like Cetirizine) if you believe an allergy might be contributing. Kindly consult your doctor and take medicines accordingly.

  3. Monitor symptoms: Seek immediate medical care if you experience worsening symptoms, such as increased redness, spreading of the swelling, or new systemic signs like chills or confusion.

I hope this helps.

Kindly revert so I can assist you further.

Thank you.
