I am a smoker for 29 years, about half-pack a day. I just went through a chest CT scan yesterday. Can you help me to review the Dicom image files to see if everything is fine?

Review of given CT chest scan images (attachment removed to protect patient identity): There is no obvious focal lesion in the lungs. There is mild hyperinflated lungs with concerns for early changes due to emphysema. This is not a serious problem. However, you might like to alter your smoking habits and decrease the chances of further lung changes.

I have been running for more than 10 years. Recently I have gone through intensive Marathon training, and finished my first Marathon six months back. Will this contribute to "There is mild hyperinflated lungs with concerns for early changes due to emphysema."? Just want to provide you some more details of my status.

Recent marathon training will not cause much changes in the lungs. Smoking half a pack of cigarettes a day for 29 years is a long span of time and probably this might be a reason. As such the lungs are not showing any serious concerns. If you are not short of breath and if the lung function tests are normal then you can continue with the marathon training.
