What could be the reason for pain in my lower left flank, blurry vision, irritability and fatigue?

Hi doctor,

I am a 43-year-old male, who is not under any medication. I wonder if I have any issues with my kidneys. I have pain in the lower left flank, my eyes feel funny, with slightly blurry vision. I am easily irritated and get tired fast. I had a blood test done, and the results came back all normal, except for high calcium and prolactin levels. I had an ultrasound done, that came back normal. My blood pressure and sugar levels are normal. I was a heavy or borderline alcoholic previously. I had these symptoms for a year.

I understand your concern.

At your age, if the calcium and creatinine levels are high, a bony lesion should be suspected. Blurry vision can be due to hyperviscosity of the blood. One differential diagnosis can be myeloma, but that needs to be confirmed by serum protein electrophoresis with a free-light chain ratio. I would suggest you get an X-ray of the lumbar spine to look for any lesions in the skull bone, hemoglobin level, and erythrocyte sedimentation rate (ESR). Other differential diagnoses combining blurry vision and high prolactin level can be pituitary gland involvement if you have difficulty in temporal vision (or lateral side vision) from both eyes. So to rule these out, first I would suggest you get serum protein electrophoresis, free light chain ratio with X-ray lumbar spine.
