Patient's Query

Hi doctor,

I regularly perform cardiovascular exercises and weight training. My protein intake daily is about 180 to 210 grams, my fiber intake is anywhere from 20 to 40 grams, and my water intake is approximately 4 liters daily. I fart a lot. I read that the average person should fart 5 to 15 times daily. I fart typically 50+ times a day, and they tend to smell. Is this normal?

I have also attached four files containing textures of my stool. Why do they differ in size and shape?

Hi,

Welcome to icliniq.com.

I reviewed your detailed history and also saw the images (attachment removed to protect the patient's identity).

This fart frequency is high. Sometimes people cannot digest fibers and carbohydrates well, which leads to such high frequency. I will recommend that you use some treatment, and it will help out.

Take Tablet Rifixamine 550 mg (antibiotic), one tablet two times daily for three weeks, and continue the good work of regular exercises and a healthy diet.

There is nothing worrisome in your history.

Patient's Query

Hi doctor,

Thank you for the reply.

So there is nothing worrisome like colon cancer or anything I should be worried about?

Hi,

Welcome back to icliniq.com.

Yes, nothing worrisome in it. What is your age?

Patient's Query

Hi doctor,

Thank you for the reply.

I am 30 years old. Also, I read on cancer forums that some people only experienced 'being gassy' and that they were diagnosed with colon cancer which makes me worried.

Hi,

Welcome back to icliniq.com.

You do not have to worry. There is nothing suggestive of cancer in your history.

Patient's Query

Hi doctor,

Thank you for the reply.

As per my photos, I cannot relate my stools to the bristol stool chart. Why is this?

Hi,

Welcome back to icliniq.com.

Your stool shapes are changing. That is why you cannot correlate it exactly. I have seen in the pictures that bristol was 4,5.

Patient's Query

Hi doctor,

Thank you for the reply.

So there is no reason to push for a colonoscopy?

Hi,

Welcome back to icliniq.com.

There is no reason for a colonoscopy.

Patient's Query

Hi doctor,

Thank you for the reply.

Are you really a Gastroenterologist? How do I know that I can trust your answer?

Hi,

Welcome back to icliniq.com.

It is very difficult to answer this question. If you have trust issues, then it can be proved only when you see me live in my endoscopy unit. Anyhow you can search for my qualification on the internet.

Patient's Query

Hi doctor,

Thank you for the reply.

I understood; just asking the question. So there is no need to spend my money to go and see a GI specialist?

I only got scared of colon cancer because my friend, who is 33 years old, was diagnosed with colon cancer, and now he has only six months to live.

Hi,

Welcome back to icliniq.com.

Yes, I do not think that you need it.

Patient's Query

Hi doctor,

Thank you for the reply.

And I have two final questions:

1. Since the Coronavirus lockdown, I have been getting diarrhea every two to three weeks. Ever since starting Sertraline 50 mg, it was every day but has now disappeared. But I notice my stomach gurgles a little more. Is this normal and related to anxiety?

2. My stools come out with ragged edges. Is this normal? Could it be suggestive of a high fiber diet?

Hi,

Welcome back to icliniq.com.

1. It is anxiety related, and it improved with Sertraline as well.

2. This stool shape is due to high fiber diet.

I will recommend taking a short course of medications mentioned previously.

Patient's Query

Hi doctor,

Thank you for the reply,

1. I have passed clear mucus in my stool sometimes. Is that a cause for concern?

2. What if the medications do not help?

Hi,

Welcome back to ciliniq.com.

It is not a cause for concern, and it can happen in irritable bowel. Medications will help. If it does not help, then we will consider for further treatment.

Patient's Query

Hi doctor,

Thank you for the reply.

I have had farting and stool issues for as long as I can remember.

Hi,

Welcome back to icliniq.com.

I am hopeful that it will improve with my prescribed treatment.

Patient's Query

Hi doctor,

Thank you for the reply.

It does not really bother me, to be honest, but maybe I should try a low FODMAP diet. Also, is there any chance that I might have colitis or IBD?

Hi,

Welcome back to icliniq.com.

Yes, you can have a trial of a Low FODMAP (fermentable oligosaccharides, oligosaccharides, monosaccharides, and polyols) diet and it is not suggestive of IBD (irritable bowel syndrome).

Patient's Query

Hi doctor,

Thank you for the reply.

I really appreciate your reply and patience. To summarise, Is stools changing day to day suggestive of what we eat?

Hi,

Welcome back to icliniq.com.

Yes I agree. I am always there for any type of support.

Patient's Query

Hi doctor,

Thank you for the reply.

One last question,

Ever since taking Sertraline, I notice that near my rectal area is like 'popping or spasming'. What could it be?

Hi,

Welcome back to icliniq.com.

Tablet Sertraline has no such adverse effects. You are anxious about these, and a minor phenomenon may look aggravated to you. It is a perineal muscle spasm. Do not worry about it.

Patient's Query

Hi doctor,

Thank you for your response.
