What can cause frequent urination and burning sensation?

Hello doctor,

Over the past three months, I have been having burning sensations and frequent urination, along with some microscopic blood in my urine. My PSA (Prostate-Specific Antigen) level is higher than normal (1.87 for a 36-year-old, with a normal value being under 1.4). I have had a few tests done a CT (computed tomography) scan of my urinary tract 1.5 months ago showed a 5 mm kidney stone in my left kidney, but everything else seemed fine. The radiologist noticed a little thickening in my bladder wall, but was not sure if it was because my bladder was not full (I went to the toilet right before the scan) or if there was another cause. They suggested I get a cystoscopy, which I did, and it came back normal. I also had a urine cytology test that was negative. Three weeks ago, I had shock wave lithotripsy (SWL) to break the kidney stone, and three days ago, I had an ultrasound that showed everything normal except for the 5 mm stone still there.

There has not been any infection in all my urine tests, but there is almost always crystallization. So, I am wondering what is causing these symptoms. Could it be bladder cancer?

I would like to know if you have undergone a digital rectal examination and whether there was any tenderness when the urologist touched your prostate. If so, this may indicate a prostatic infection or inflammation. Your elevated prostate-specific antigen also suggests that there may be ongoing inflammation in your prostate.

I would advise you to consider treatment for prostatitis, which usually involves a course of prostate-penetrating antibiotics like Fluoroquinolones and an alpha blocker (a medication that relaxes the bladder neck, and prostate) for four to six weeks. After that, you should be reassessed. Many patients benefit from this first-line treatment. However, if the pain persists, you may need to explore second-line treatment options. Your risk of having cancer is very low, as you have been investigated for it and found to be normal. Please do not worry about that.

Blood in the urine can be caused by either a kidney stone or a prostate infection. There is no need to worry about the 5 mm kidney stone, as it can be managed medically. I would advise you to start antibiotics and alpha blockers in consultation with your local urologist.
