Please can you interpret my recent laboratory report? Why is my hemoglobin dropping all the time? Why is my blood report abnormal? Do you think I have blood cancer? Or any other cancer? Do you want me to go to an oncologist?

I am giving you my opinion regarding your reports (the attachment was removed to protect the patient's identity).

Your CBC is showing microcytic hypochromic anemia. Anemia means a low hemoglobin level; the oxygen-carrying capacity of blood is affected. Your reticulocyte count is high, and polychromasia is present with high bilirubin. So you can have hemolytic anemia and iron deficiency anemia combined.

I suggest the following investigation for you:

  1. Serum ferritin.
  2. Serum TIBC (total iron binding capacity).
  3. Serum transferrin receptor saturation.
  4. Hb electrophoresis by the HPLC method.
  5. G6PD testing.

According to reports, a further management plan can be decided. You will need an iron tablet or injection to improve your hemoglobin level, but the above reports have to be done to find the cause. In hemolytic anemia, RBC destruction occurs, and the underlying cause is searched for. Your report does not show any cancer signs, so do not worry.
