Hello doctor,

My mother is feeling weak. She has pain in her left hip joint, thigh and leg. She also feels cold. We took hemoglobin and thyroid profile test. The result is as follows. Hemoglobin 7.4 (reference interval : 12 - 15), total T3 91.4 (reference interval : 70 - 204), total T4 5.8 (reference interval : 5.5 - 11.0) and TSH 13.72 (reference interval : 0.35 - 5.5). Could you please suggest?

She has an underactive thyroid. Does she have any history of this problem? This is an endocrine issue and she needs treatment. You have not provided her weight for dosage recommendations. Her hemoglobin is quite low and this is not an endocrine issue. Is she consulting with someone for this? Is she bleeding from anywhere? Both these problems can cause the symptoms you describe. How are the blood pressure and pulse rate?

Thank you doctor,

She did not have any thyroid problem previously. She had iron deficiency problem for the last three years and for which she had medication. She is not bleeding from anywhere. Currently, she is not taking any medication. Her weight is 116.8 lbs. Her BP is 113/74 mmHg and pulse is 76.

I suggest her 75 mcg Levothyroxine on an empty stomach. It should be separated from Iron tablet by at least 4 hours. Consult her doctor, discuss with him or her and take the medicine with consent Please get another TSH (thyroid-stimulating hormone) alone in six weeks. If the iron problem has not resolved, then it is time to see a hematologist. Hemoglobin of 7 is very low.
