Hi doctor,

I had a routine laboratory test done with my physician last week. My lymphocytes are elevated, neutrophil count seems low, and liver enzymes (AST and ALT) are elevated. I have no fever or weight loss.

Recent clinical history: before 47 days, I took tablet Flagyl 500 mg twice a day for seven days for vaginal infection, one dose of Diflucan that day, and one dose of Diflucan after eight days.

Before 22 days, I had a severe sore throat and an enlarged lymph node on the left side of the neck with no fever or congestion. Then after one week, I had an appoinment with PCP as the pain persisted. The doctor prescribed Amoxicillin 500 mg empirically, even though it was presumed viral. I only took three doses as the pain was improving. Then I had vaginal itching, which I assumed was a side effect from Amoxicillin, and I did not finish the entire antibiotic course. So, I took one dose of Diflucan. The symptoms of sore throat resolved before 11 to 12 days.

I have had no other sicknesses since then. I took three doses of Ibuprofen in the past week due to headaches (I was attempting to decrease my caffeine consumption, which caused headaches). No other medications were taken. I am taking vitamin D and vitamin C daily.

Would you please advise regarding abnormal laboratory reports? When I searched on the internet, it showed as leukemia.

Following is my opinion on the questions you asked. In the attached reports (attachments are removed to protect the patient's identity), lymphocyte count is high, SGPT (serum glutamic pyruvic transaminase) is also high. It points more towards viral upper respiratory infection. You have a history of sore throat as well. You can check your lymphocyte count after one or two weeks. If your lymphocyte is still high after two weeks, then I suggest you flow cytometry.

You need not worry about leukemia only. Mostly elevated lymphocytes will subside after one week. Your hemoglobin and hematocrit levels are also low, suggesting mild anemia, for which I would suggest Ferrous ascorbate tablet.

Hi doctor,

Thank you for the quick reply. So the fact that my WBC is normal could still mean viral infection? Also, SGPT is the same as ALT, right? Can the liver enzyme be elevated with a respiratory infection? What about the fact my SGOT is elevated too?

I did send a message to my doctor's office, but it could be days for them to respond, so I appreciate you responding.

The following is my opinion on the questions you asked. Even with a normal WBC count, a viral infection could be possible if lymphocyte count is elevated. However, repeat it after one to two weeks, as I suggested. If, after two weeks, the lymphocyte count is still high, then flow cytometry can be done further.

Your SGPT and SGOT are high but are not twofold elevated. In liver inflammation, SGOT and SGPT can be increased, and they are elevated twofold, especially more than 84. In your case, it is below 80, So it does not seem like liver inflammation. However, after two weeks, along with CBC (complete blood count), repeat SGPT as well.
