What could cause high testosterone levels and an enlarged prostate?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I recently underwent lab tests, and the results indicated a high testosterone level (attached lab report). This is not the first time; my previous lab work also showed elevated testosterone levels. Despite this, my primary doctor did not express concern about the heightened levels. I have been diagnosed with an enlarged prostate (attached report), and I cannot shake the feeling that my testosterone levels are increasing. Also, I take losartan for my high blood pressure issues. However, my doctor has not explained or expressed any concerns. Should I worry? Please help.

Thank you.

Patient's Query

Hi doctor,

Thank you for your reply.

I do not have diabetes, but I do have high blood pressure, indicating hypertension. In the past, I underwent spinal fusion surgery. Four years ago, I encountered a prostate infection accompanied by blood in the urine and elevated PSA levels. Fortunately, the condition was effectively treated with antibiotics, returning my PSA levels to normal. Presently, I do not experience any difficulties passing urine, and there is no straining. However, I have observed a decrease in my ability to hold urine for as long as I used to. It is worth noting that I am a non-smoker. Kindly guide.

Thank you.

Hi,

Welcome back to icliniq.com.

I read your query and can understand your concern.

I have thoroughly examined your report set (attachments removed to protect the patient's identity). Overall, everything appears to be in order, with the exception of elevated testosterone and a low vitamin D level.

Elevated testosterone levels can be attributed to various factors:

  1. Some men naturally have higher testosterone levels.

  2. Adrenal tumors (abnormal growths in the adrenal glands).

  3. Cushing syndrome is characterized by an excess of cortisol.

  4. Thyroid disorders (although your thyroid workup appears normal).

  5. Previous use of testosterone-increasing medications.

  6. Men on steroids for other reasons.

It is worth noting that your testosterone levels are not excessively high. Nevertheless, I suggest obtaining an ultrasound to ensure the well-being of your adrenal glands and to assess the current status of your prostate. This will aid in making informed decisions about any necessary next steps.

I hope this helps.

Please revert in case of further queries.

Thank you.
