Hello doctor,

My dad has a persistent blood pressure of 150/95 mmHg even after taking Telmisartan 40 in the morning and Cilnidipine 20 in the evening. What should be done, and how risky is this blood pressure?

I understand your concern.

Your dad is taking Telmisartan 40 mg once a day. If he has been on it for over two weeks and his kidney function is stable, the dose can be increased to 80 mg, either as a single dose in the morning or as a divided dose (40 mg in the morning and 40 mg in the evening). He should continue taking Cilnidipine 20 mg. The ideal blood pressure should be less than 130 mmHg for systolic (top number) and less than 85 mmHg for diastolic (bottom number). If the blood pressure is not controlled according to these recommendations, other medications may need to be added.
