Hello doctor,

My wife and I have been trying to conceive for almost 18 months now, with no success. She is 28 and I am 33, and neither of us has any history of pregnancies or known fertility issues. We have been timing everything correctly, and she tracks her ovulation religiously. We are both getting really stressed about this.

What kinds of treatments should we be looking into at this point? Are there specific lifestyle changes or dietary adjustments that could help improve our chances? We are willing to try anything before considering more invasive procedures.

If you have been trying to conceive for over a year with regular, unprotected intercourse and have not had success, it is time to begin investigating potential causes. It is important to assess both partners to identify any underlying issues contributing to infertility. Before proceeding with tests, I would like to ask a few history-related questions:

For your wife: Has there been any history of:

  1. Unexplained weight gain, acne, or excessive hair growth?
  2. Symptoms like cold intolerance, palpitations, excessive sweating, tremors, or a fast heartbeat?
  3. Headaches, blurred vision, nausea, vomiting, or nipple discharge?
  4. Urinary symptoms such as burning, urgency, increased frequency, or discomfort, as well as hot flashes or vaginal dryness?
  5. Persistent dry cough, unexplained weight loss, fever, or night sweats?

For you (the husband):

  1. Do you smoke?
  2. Have you had any previous spinal or pelvic surgery?
  3. Any past trauma to the spine or pelvic region?
  4. Any sexual health concerns, such as premature ejaculation or erectile dysfunction?

Recommended investigations:

For your wife:

  1. Serum FSH (follicle-stimulating hormone) and LH (luteinizing hormone) on the second day of her menstrual cycle.
  2. Serum prolactin.
  3. Thyroid function tests.
  4. Pelvic ultrasound.

For the husband:

  1. Semen analysis.

Management plan:

  1. First and foremost, try not to stress; emotional well-being plays a significant role in fertility.
  2. Aim for regular sexual intercourse at least twice a week throughout the cycle.
  3. Begin taking 5 mg of Folic acid once daily for three months.
  4. Consider starting Inomel-F sachets (Folic acid and Myoinositol), one sachet at night for three months, which may support fertility. Consult your specialist doctor, discuss with him or her, and take the medicines with their consent.

Once the investigations are complete, please share the test results with me. Based on the findings, we can discuss the next steps.

Possible treatment options (if needed):

  1. Ovulation induction medications, such as Clomiphene citrate 50 mg or Letrozole.
  2. If ovulation induction is unsuccessful, the next steps may include gonadotropin injections, intrauterine insemination (IUI), intracytoplasmic sperm injection (ICSI), and in vitro fertilization (IVF).
