Should Progesterone doses be increased in the Letrozole cycle if menstruation occurs before stopping Progesterone?

Hello doctor,

I am currently undergoing my fifth timed intercourse cycle with Letrozole (5 mg, day 3 to 7), 7500 hCG trigger, and follicle monitoring. I am taking oral micronized Progesterone 500 mg daily (200 and 300 mg) from the day of the trigger shot. My query is that I have read that you get your period a few days after stopping Progesterone. However, for all my Letrozole cycles, I get my period on day 28 while I am still taking Progesterone pills which my doctor prescribed until the 30th day. Should I increase my dose? My doctor does not want to increase the dose due to possible side effects.

The protocol is fine, and the Progesterone dosage is adequate. If the cycle is successful, you will not menstruate, but if it is unsuccessful, then you may bleed ideally after the stoppage of Progesterones, but you have spotting prior to the start of bleeding even on day 13 or 14 after the hCG trigger.

Thank you very much for the response. Also, should Progesterone be started a few days after ovulation? Why does my doctor ask me to start it on the day of the trigger along with Estradiol Valerate (2 mg twice for six days)?

If the follicular monitoring suggests that the size of the ovum is more than 20 mm, then ideally, after 36 hours post hCG trigger, ovulation is expected, and Progesterone can be started accordingly. However, protocols are tailor-made and vary from woman to woman.
