Why am I bleeding if my pregnancy test was positive?

Hello doctor,

My period was late by two days. It was supposed to come last month, but it did not. This month as well, it did not come. I bought a pregnancy test kit, and when I checked, I saw a faint line, so it seemed positive. But this morning, I started bleeding.

I am around six weeks pregnant. Yesterday, I accidentally slipped and fell. I am also taking tablets for depression. Last month, I took medicine to delay my period. Now, I have started bleeding.

The most likely diagnosis is early pregnancy with a threatened miscarriage or very early pregnancy loss. You had missed periods and a faint positive pregnancy test, which suggests pregnancy. Around six weeks, bleeding after a positive test is most commonly due to either a threatened miscarriage or a chemical pregnancy. Although a fall can cause anxiety, most miscarriages in early pregnancy are due to chromosomal reasons and not minor trauma.

Since you have started bleeding, it may be a threatened miscarriage if the bleeding is light and the pregnancy continues, or an early miscarriage if the bleeding becomes heavy, with clots and cramping. Medicines taken to delay your period last month may have affected your cycle timing, but they do not cause a positive pregnancy test. Depression tablets usually do not directly cause miscarriage, but the exact medication name is important.

At this stage, if the bleeding is light and there is no severe pain, you need an urgent ultrasound and a blood beta-hCG test (beta-human chorionic gonadotropin test) to check pregnancy viability. However, if the bleeding becomes heavy, soaking pads, or if you develop severe lower abdominal pain, dizziness, or fainting, you should go to the emergency department immediately. You need a medical evaluation as soon as possible to confirm whether the pregnancy is continuing or not.
