How can chronic ischemic heart disease be managed?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I would like to ask for a second opinion on the treatment prescribed by a cardiologist for my 53-year-old mother. She was diagnosed with chronic ischemic heart disease following a routine ECG, which I would like to send you, and blood tests indicated a cholesterol level of 280 mg/dL. The symptoms are not very obvious; she reports having had two to three bruises in the heart area over the last few years. The prescribed treatment includes Pentaerythritol tetranitrate (two per day), Aspenter (one per day), Rosuvastatin (one per day), Plavix (one per day), and Metoprolol succinate 47.5 mg (two per day). A Doppler ultrasound was normal, and only the ECG showed abnormalities.

Thank you in advance.

Hello,

Welcome to icliniq.com.

I reviewed the therapy prescribed by the doctor, and I agree with it. It includes a blood thinner (Aspirin and Clopidogrel) to help prevent blood clots in the coronary arteries, a beta blocker to lower the heart rate, a nitrate to help increase blood flow in the coronary arteries, and a statin to help lower cholesterol levels. I would recommend reducing the dose of Metoprolol if her heart rate is very low (below 60 BPM) or if she is experiencing chronic fatigue.

I hope you find this answer helpful.

Regards.
