What is the reason for itchy skin during early pregnancy?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hi doctor,

I am in my seventh week of pregnancy, and I am experiencing itchy skin, especially after eating, which started only in the fifth week of pregnancy. It started with itchy palms and now the whole body. I take no medications for anything, and I have no medical history of any type. I had my gallbladder removed years ago. What is the reason for my itchy skin?

Please help me.

Thanks.

Hi,

Welcome to icliniq.com.

Thank you for sharing your concerns.

Based on the symptoms you’ve described, one possible explanation could be obstetric cholestasis, also known as intrahepatic cholestasis of pregnancy. This is a liver condition that can develop during pregnancy and is commonly associated with significant itching, particularly on the palms of the hands and soles of the feet. Although the itching can be very uncomfortable, it is a recognized condition and can be managed with proper care.

It is important to understand that this condition may recur in future pregnancies, with recurrence rates reported to be high. While obstetric cholestasis cannot be completely cured during pregnancy, the symptoms and associated risks can usually be controlled effectively through appropriate treatment and regular monitoring.

To better evaluate your condition and support an accurate diagnosis, the following investigations are recommended:

  • Complete blood count to assess your overall health.
  • Midstream urine analysis to rule out infection.
  • Fasting blood sugar to evaluate glucose levels.

Viral screening tests:

  • Hepatitis B surface antigen.
  • Anti-hepatitis C virus test.

Liver-related assessments:

  • Liver function tests to evaluate liver health.
  • Serum bile acid levels, which are particularly important in suspected cholestasis.
  • Obstetric ultrasound to assess the baby’s well-being.

For symptomatic relief and management, your doctor may consider:

  • Calamine lotion is applied twice daily to soothe itching.
  • Clobetasol propionate cream is applied thinly to affected areas for short-term use if prescribed.
  • Cetirizine is taken for a few days to help reduce itching.
  • Ursodeoxycholic acid, which is commonly used to help lower bile acid levels and improve symptoms.

Please try not to be overly anxious. Many patients with this condition have healthy pregnancies when they receive timely treatment and regular follow-up. Close monitoring is important to ensure both your comfort and the baby’s safety.

If your itching becomes more severe or if you notice any new symptoms, it is advisable to consult your healthcare provider promptly. If you have any further questions or need clarification, feel free to ask. I am here to help.

I hope this answers your query.

Thank you.
