Hello doctor,

I have itching in my private area, and it becomes worse at night. The rash looks like a map spreading on the skin and is very irritating. It started about two weeks ago as a small pimple, and now it has grown to about the size of one finger space.

What can I do, and which cream or ointment would be good for it?

Based on the picture and your history, this appears to be a fungal infection called tinea cruris. With the right topical medication and proper aftercare, it will heal. There is no need to worry, as it is a highly manageable condition.

To manage your condition, I shall suggest you take a few medications. Please consult your specialist doctor, discuss the matter with them, and take the medicines only with their consent.

  • Take Cetirizine 10 mg once at bedtime. If the itching is severe, you may take it both in the morning and at night.

  • For topical treatment, apply Hydrozole cream (Clotrimazole and Hydrocortisone) at bedtime for seven days.

  • After seven days, switch to Terbinafine cream and continue using it for an additional two weeks. Even if the infection or bump disappears within the first seven days, please still switch to Terbinafine and continue treatment for two weeks, as fungal infections tend to recur.

To further manage your condition, please follow these care instructions:

  • Keep the area clean and dry.

  • Avoid tight clothing and minimize sweating in the area.

  • Do not scratch, as this can worsen the infection.

  • Avoid sharing towels or clothing, as the infection can spread to others.
