What are the cause of patched thumbnail and itchy body?

Hello doctor,
I am a 28-year-old female who weighs 165 pounds and is 5 feet and 4 inches tall. I currently have a few issues. My thumb nail has a weird brien patch that randomly appeared a few days ago 9mm wide and with a 6mm length- shaped surrounding. Breakouts all over the body face, legs, shoulders, arms, legs, back, bum, neck, collar bone, and chest. I also get quite itchy and have heaps of little lump-like simple feeling things over my scalp that hurt. I take 15 mg of Dexamphetamine daily and 20 mg of Lexapro and Ventolin.
Please help.
