How do I manage itchy scalp and flaky eyelids?

This Premium Q&A, reviewed and published, features a real conversation between an iCliniq user and a physician.

Patient's Query

Hello doctor,

I have been experiencing an itchy scalp for one week, along with dry, puffy, and flaky eyelids. The itching is worse at night.

Please help.

Hello,

Welcome to icliniq.com.

Of course, I am aware of how annoying a case of itchiness and flakiness along with puffiness around the eyes can get, especially if the symptom aggravates at night. This can really disrupt your sleep and comfort in general. From what you have described, it is highly likely to be a case of seborrheic dermatitis, which is a rather common inflammation of the skin.

Seborrheic dermatitis is a widespread, non-infectious type of inflammation of the skin that affects areas of the body with a lot of oil glands, for instance, the scalp, eyebrows, the sides of the nose, the ears, and the eyelids. This problem often entails the symptoms of itchiness, redness, greasiness/dry flakes, and mild swelling.

The exact cause is not related to poor hygiene. The disorder arises as a result of an exaggerated reaction of the skin to a fungus that occurs naturally on the skin, known as Malassezia. It may be caused by many different causes, such as stress, inadequate sleep, changes in the weather and hormonal imbalance. That is why symptoms sometimes flare up suddenly.

Do not worry, with the right treatment, it is very manageable. I am going to give you step-by-step guidance to help manage your condition effectively.

For the scalp, I recommend using 2 percent Ketoconazole shampoo three times per week. Apply it to the scalp, leave it on for approximately five minutes, and then wash off thoroughly. On other days, you may use a mild regular shampoo. This will help reduce yeast overgrowth and inflammation, contributing to itching and scaling.

At bedtime, apply Betnovate lotion (containing Betamethasone) to the affected scalp areas once daily for two weeks only. Use a small amount. You can gently massage it into the itchy patches. This will quickly reduce inflammation and itching. Do not continue beyond two weeks unless reviewed, as prolonged steroid use on the scalp is not advised.

For the eyelids, apply a very thin layer of 1 percent Hydrocortisone cream carefully at bedtime for only seven days. Make sure it does not enter the eyes. Use the smallest amount necessary and stop after one week.

Moreover, you should not use harsh soaps on your face, should use a mild soap instead, and should not rub your eyelids. Seborrheic dermatitis may be worsened by stress and lack of sleep.

I hope this has helped you.

Please feel free to reach out to me again if you have further queries.

Thank you.
